Housing Field Checklists

Use these short, printable lists during housing instability or shelter intake.

First Night

  • Call the Veteran homelessness hotline or local access point.
  • Confirm intake time before traveling.
  • Pack identification, medications, phone charger, water, and essential documents.
  • Tell a trusted person where you are going.
  • Identify one backup location.

Shelter Intake

  • Photo ID and Veteran identification, if available.
  • Medication list and medically necessary equipment.
  • Household names, ages, and relationship information.
  • Ask about storage, curfew, transportation, pets, and accessibility.
  • Write down staff names and next appointments.

Vehicle Living Safety

  • Never sleep in a running vehicle in an enclosed area.
  • Carry drinking water and monitor heat exposure.
  • Keep the phone charged and emergency contacts accessible.
  • Store medications within required temperature limits.
  • Begin coordinated-entry or Veteran housing contact today.

Eviction Response

  • Photograph every page of the notice.
  • Write down the court, response, and lockout deadlines.
  • Contact legal aid and rental-assistance screening promptly.
  • Gather lease, rent ledger, proof of payment, income, and communications.
  • Make a backup housing plan while seeking assistance.

Apartment Application

  • Identification and Social Security documentation.
  • Income, benefit, or subsidy verification.
  • Rental history and references.
  • Application fees and deposit requirements confirmed in writing.
  • Accessibility and reasonable-accommodation needs documented.

  • Photo identification and Veteran ID.
  • DD Form 214 or access to a digital copy.
  • Benefit letters, pay stubs, and bank information.
